Lee County Medical Society Alliance Foundation MINI-GRANT REQUEST ELIGIBILITY REQUIREMENTS The Organization agrees that it complies or will comply with the following: 1. It must be based in Lee County or be the Lee County affiliate of a parent organization and must guarantee that a majority of the donated funds will remain in Lee County to serve the citizens of Lee County; or If a state or national organization with no organized Lee County affiliate, it must guarantee that 100% of the donated funds will remain in Lee County to serve the citizens of Lee County. 2. It must demonstrate that the project or need is directly related to health education or the treatment, prevention or rehabilitation of physical or mental disease, illness or injury. 3. It must be willing to promote the LCMSAF, the project/need that the funds are requested for, and the individual fundraising activity to all of their members, and other community organizations. 4. It must be willing to give updates on the project, including the status of other funding and the projected time frame on a periodic basis or at the request of the LCMSAF. 5. It must allow periodic site inspections, if requested. 6. It must agree to respect and follow all rules and regulations surrounding this process. 7. It must provide a current and complete list of their Board of Directors. 8. It must use the funds for the project, as presented, within one (1) year of receipt. |
